Senior Community Connector
Helping seniors connect to community resources
and plan for the future
The Seniors Community Connector offers free, confidential support to those age 55+ to navigate community resources and activities, find community and explore options regarding Advanced Care Planning, and more. The Connector also liaises and collaborates with other service organizations in the community to connect you with ongoing support.
We provide seniors and elders with client-focused support that is tailored to their situation. We connect you with community services to meet your social, emotional and/or physical needs.
client process:
Step one: Referral
- Individuals can refer themselves to the program or receive a referral from a health care professional.
Step two: Intake Appointment
- We will contact you within three business days to book an appointment
- At the intake appointment we will get to know you and explore options for enhancing your health and wellbeing through community resources.
- We will identify goals and make a plan to achieve them.
Step three: Continuing Support
- We will continue to support you while we work on your goals and refer you to other support in the community
